You will work directly with the adolescent foster care population, creating personalized care plans, identifying necessary resources, and coordinating services to ensure high-quality, cost-effective care. **You might be a good fit if you...** - Possess a Master's degree in Behavioral Health or Social Work, or are an RN with a degree from an Accredited School of Nursing and 2-4 years of related experience. - Hold a relevant professional license (LCSW, LMSW, LMFT, LMHC, LPC, or RN with BH experience). - Are skilled in assessing member needs, developing care plans, and coordinating with providers and community resources. - Can effectively communicate with members and their families about treatment options and healthcare benefits, with a focus on mental health and substance use disorders.

POSITION IS REMOTE BUT CANDIDATE MUST RESIDE IN STATE OF MISSOURIPOSITION WILL WORK WITH ADOLESCENT FOSTER CARE POPULATIONPosition Purpose: Develops, assesses, and facilitates complex care management activities for primarily mental and behavioral health needs members to provide high quality, cost-effective healthcare outcomes including personalized care plans and education for members and their families related to mental health and substance use disorder.












Education/Experience: Requires a Master's degree in Behavioral Health or Social Work or a Degree from an Accredited School of Nursing and 2 – 4 years of related experience.

Pay Range: $56,200.00 - $101,000.00 per year
