This mid-level role is designed for healthcare professionals with a Bachelor’s degree and 2–4 years of experience in care coordination or social services. It is an ideal fit for those with a background in Medicaid, health advocacy, or psychosocial support who are comfortable navigating community resources like housing, education, and food assistance. While clinical licensure (RN) is a plus, it is not strictly required.

This is a fully remote position offering a stable Monday–Friday schedule (8:00 AM – 4:30 PM).
