This mid-level role is specifically designed for healthcare professionals with a background in postpartum care who reside in Florida. The ideal candidate will be an LPN (Licensed Practical Nurse) or hold a Bachelor’s degree with 2–4 years of experience in care coordination, case management, or social services. You’ll be the primary point of contact for members, helping them navigate complex healthcare landscapes and social obstacles to improve their recovery and wellness. This is a 100% remote position with very minimal travel (only 1–2 times per year), offering a consistent Monday–Friday, 8 AM – 5 PM EST schedule.
