Work type: remote

Location: Remote-WA State

Type: Full-time

This role is ideal for social service or healthcare professionals with a Bachelor’s degree and 2–4 years of experience in care coordination. While it is classified as "remote-WA," it is specifically for those residing in **King County** who are comfortable with high-intensity field work. A clinical license is a plus but not strictly required if you have equivalent experience. The position offers a unique "field-remote" hybrid balance: you’ll work from home for administrative tasks but spend **50%–75% of your time traveling** within Seattle and surrounding areas to meet members where they are—including homes, shelters, and hospitals. Centene provides a comprehensive benefits package, including a 401k, stock purchase plans, and tuition reimbursement. **You might be a good fit if you:** * Live in Seattle, Tukwila, SeaTac, Burien, or Renton and know the local community resources well. * Are passionate about addressing social determinants of health like food insecurity and housing. * Are comfortable conducting both phone outreach and in-person visits in diverse settings. * Have experience managing personalized care plans for high-risk populations.
