Summary

You'll need an active pharmacist license in at least two of the specified states and be willing to get more. Aledade is offering a full-time, fully remote position with a salary range of $96,000 to $135,000 per year. You'll be optimizing patient outcomes through telephonic interventions, focusing on medication management. * Valid pharmacist license in at least two of the following states: AL, CA, DE, KY, LA, MS, NC, OH, PA, UT, VA, WV, with a willingness to acquire more. * Experience with telephony platforms and Electronic Health Record (EHR) systems. * In-depth knowledge of pharmacotherapy and pharmacy practice. * Familiarity with Medicare, HEDIS, and STAR ratings.

This role focuses on the tactical execution of medication management services, including polypharmacy, adherence, and pharmacotherapy optimization.

Candidates should be comfortable working remotely from anywhere within the US, across all US time zones.
