**Who this is for** This position is for an individual residing in North Carolina who is interested in supporting members by connecting them to vital community resources and social services to improve their overall health outcomes. **Key highlights** As a Community Resource Coordinator, you will act as a bridge between clinical care teams and members, conducting non-clinical assessments and outreach. This role requires travel within specific North Carolina counties to provide hands-on assistance, home safety evaluations, and community-based support. **You might be a good fit if you...** - Have at least 1–2 years of related experience and a high school diploma or GED. - Understand the impact of Social Determinants of Health (SDOH) on member wellbeing. - Are comfortable performing field work, including visiting homes and community organizations to assist hard-to-reach families. - Possess strong documentation skills and the ability to provide informal guidance to members on available health and social services.

Must reside in the state of North Carolina for travel within some of the listed counties.(Bladen, Brunswick, Columbus, Cumberland, Harnett, Hoke, Lee, Montgomery, Moore, New Hanover, Pender, Richmond, Robeson, Sampson, and Scotland County)Position Purpose: Supports community connection activities including connecting members to community resources to support their care management journey and provide necessary care resources in a cost-effective manner. Provides members with known community resources and supports the care team to identify member community support and provide health education as appropriate.
Pay Range: $17.84 - $28.02 per hour
