This mid-level role is designed for community-minded professionals with a background in foster care and a deep understanding of Social Determinants of Health (SDOH). You are the ideal candidate if you enjoy fieldwork and are based in Central Kansas (Saline, Ellsworth, McPherson, or Rice Counties). A High School diploma and 1–2 years of experience are required, and while a Community Health Worker certification is preferred, the company will support you in obtaining it within your first year.
The position offers a unique "Remote-KS" designation, but it is heavily field-based with 75% travel for in-person member visits. The schedule is a fixed midday/evening shift (M-F, 10 AM – 7 PM), making it a great fit for those who prefer non-traditional hours.
