This fully remote position requires some travel within Story County, with mileage reimbursement provided. You'll work Monday to Friday, 8 am to 5 pm, and will be part of an organization dedicated to transforming community health. **You might be a good fit if you...** - Have a Bachelor's degree and 1+ years of experience with populations served, or are an RN with 6+ years of experience with the population served. - Can evaluate member needs, assess available resources, and coordinate the best possible outcomes. - Are skilled at coordinating between members, families, caregivers, and care providers to ensure services are accessible. - Possess the ability to monitor care plans, member status, and outcomes, making recommendations for improvement.

This role requires approximately 75% travel to support members in Story County. Applicants have the flexibility to work remotely from their home the remaining time. We provide all required equipment and reimburse for mileage at the current IRS rate. The schedule is Monday - Friday, 8am - 5pm.
Position Purpose: Assists in developing, assessing, and coordinating holistic care management activities to enable quality, cost-effective healthcare outcomes. May develop or assist with developing personalized service care plans/service plans for long-term care members and educates members and their families/caregivers on services and benefits available to meet member needs.
Pay Range: $22.94 - $38.79 per hour
