Work type: remote
Location: Remote-MS
Salary: $27 – $49/hr
Type: Full-time
This role is ideal for community-focused healthcare professionals with a background in social work, public health, or nursing. You should have at least two years of experience in community health and a solid understanding of Medicaid/Medicare programs. While technically "remote," this is a field-based position requiring residency in or near specific Mississippi counties (Forrest, Hinds, Bolivar, or Lauderdale) to conduct home visits and local outreach.
The position offers a competitive hourly rate with a broad range based on experience, backed by a comprehensive corporate benefits package including stock purchase plans and tuition reimbursement. It balances the autonomy of field work with the stability of a national healthcare organization. You will act as a critical bridge between vulnerable members and the healthcare system, focusing on closing care gaps and addressing social determinants of health.
**You might be a good fit if you...**
* Reside in Mississippi near the target counties and are comfortable with local travel and home visits.
* Have experience navigating HEDIS or CAHPS quality measures within a health plan environment.
* Are a licensed RN or LCSW looking to move into a community advocacy and quality improvement role.
* Enjoy "boots on the ground" work, helping members schedule appointments and navigate complex social services.
