This role is designed for advanced clinical professionals—specifically Registered Nurses or Master’s-level Behavioral Health practitioners (LCSW, LMSW, LPHC, etc.)—with 4 to 6 years of experience. You will manage high-acuity caseloads involving complex mental health and social needs, acting as a critical bridge between patients, providers, and community resources. While listed as remote, this is a **hybrid field-based role** requiring up to 50% travel for home visits within Wayne or Macomb Counties, Michigan.
